Background:
Recently the H7 dTMS coil was FDA approved for adjunctive treatment of adults with OCD based a multicenter sham-controlled study 100 adults. Will these findings hold up in a naturalistic setting?
Objective:
Retrospective chart review of adolescent and adult patients who received dTMS with the H7 for OCD in three outpatient clinics in south Florida
Methods:
Twenty-nine patients with medication resistant OCD were administered dTMS with the H7 coil over the mPFC/ACC five times weekly at 100%MT, 20Hz, 2 second trains, 20 second intervals for 2000 total pulses. The treatment was administered immediately after provoking the patient’s specific OCD symptoms in an individualized tailored fashion. YBOCS were administered at baseline, weekly, and endpoint; response was defined as 30% decrease from baseline, partial response defined as >20% decrease, and remission defined as a YBOCS<10.
Results:
Patients’ ages ranged from 15 to 70 years old; seven were under the age of 22. Patients received a range from 11 to 53 treatments. Of the 29 patients, 20 responded, 24 partially responded, 8 remitted, and 5 failed to respond. More treatments significantly increase (p=0.049) the mean change in YBOCS (-9.7 if < 29 treatments and -15.5 if >29 treatments). Thus, >29 sessions resulted in a significantly higher YBOCS reduction than patients with < 29 treatments. No serious adverse events were reported.
Conclusions:
dTMS with the H7 coil for OCD is efficacious for adults and adolescents in a trained general psychiatry practice setting. Further post marketing data collection is warranted to evaluate if longer treatment courses will increase response and remission rates.
