Controlled trial of 1 Hz Deep transcranial magnetic stimulation for treatment of temporal lobe epilepsy

We will test whether a novel form of Brainsway® deep transcranial magnetic stimulation (Deep TMS), has the possibility to be a safe noninvasive method for stimulating the brain, suppresses seizures in temporal lobe epilepsy (TLE).  Conventional Deep TMS can reduce cortical excitability, but not in the temporal lobes.  Here, we hypothesize that Deep TMS with the Deep TMS Coil which could stimulate deep brain structures could possibly safely reduce seizures in TLE patients.

We aim to determine whether repetitive Deep TMS could be effective in reducing seizure frequency in adult and pediatric temporal lobe epilepsy (TLE).We hypothesize that 1 Hz Deep TMS delivered over a seizure focus in 10 daily 30-minute sessions will (1) reduce seizure frequency, and (2) reduce the frequency of epileptiform abnormalities on the patients’ EEGs. We based this hypothesis on published trials showing an encouraging antiepileptic effect of 1 Hz Deep TMS, in neocortical (extratemporal) epilepsy.

In the present study, we aim to test a novel Deep TMS coil which is unique in its potential to stimulate deep cerebral structures, including those of human temporal lobes.
